Every time you call your insurance carrier, ask for their response in writing. If by any chance they will not provide you with that information, ask who you’re speaking to, their first name and last name, their ID number, location, and keep notes of the entire conversation, including the date and time you spoke with them. This information is so important if you need to appeal a denial.
